Hudson Memorial School now uses Arbiter (formerly Family ID) to collect all online forms. You may use the following link to fill out all forms. All forms need to be completed before the student will be allowed to participate in any HMS Sports.
Concussion Testing - Students playing in a contact sport will have to complete ImPact Testing in order to create a baseline score to compare to should they get a concussion while playing. This baseline will be used to make sure the student is cleared to compete after the medical professionals has sent in letters saying they are good to play. This testing will be done at school.
