Registration
We are very excited to announce that the Alvirne Athletic Department and Hudson Memorial School have partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

We are asking the parents/guardians of all students planning to play sports to create accounts in FinalForms and register your student(s).

Before You Register
Physical Exams
A physical exam must be current prior to the start of freshman and junior year and signed by the physician. Physicals may be emailed to the Athletic Director or dropped off/mailed to the athletic office. They will be filed with the nurse once we have reviewed them. If the student's physical expires during the season, they will need to get a physical asap after it expires.
Information Needed to Register
It will be helpful to have the following information handy to allow for accurate completion of your online registration.
- Health insurance card
- Primary care doctor's contact Information
- Your child's medical history including allergies & medications
